Description: A framework for building analytic cohorts with full
        provenance. Supports branched cohort trees, per-step exclusion
        logging with human-readable reasons, schema validation at
        branch boundaries, cached derived artifacts on each cohort, and
        CONSORT diagram generation. The framework cleanly separates
        cohort definition from analysis: it produces analytic data
        tables that downstream packages can consume. A shared-base
        storage strategy keeps memory flat as the cohort tree grows.
